BACKGROUND: Diabetes Mellitus is associated with disturbances in male reproductive function and fertility. Studies have shown that oxidative stress with the subsequent inflammation and apoptosis cause these complications in diabetes. Garlic (G) (Allium sativum L) and Citrullus colocynthis (L.) Schrad (C) both have antidiabetic and antioxidant properties. Recently, we demonstrated their synergistic effects in alleviating reproductive complications when administered concomitantly. However, as even medicinal plants in long term usage may lead to some unwanted side effects of their own, we examined whether with half the original doses of these two medicinal plants we could achieve the desired results. METHODS: Thirty-five male Wistar rats were divided into five groups (n = 7/group): Control, Diabetic, Diabetic + G (0.5 ml/100 g BW), Diabetic + C (5 mg/kg BW) and Diabetic + GC (0.5 ml/100 g BW of garlic and 5 mg/kg BW of C. colocynthis) groups. The experimental period was 30 days. RESULTS: Oxidative stress, advanced glycation end products (AGEs), immunoexpression of caspase-3, and expression of mRNAs for receptor for advanced glycation end products (RAGE), NADPH oxidase-4 (NOX-4) and nuclear factor kappa B increased in testis of diabetic rats. Treatment with garlic and C. colocynthis alone showed some beneficial effects, but in the combination form the effectiveness was more profound. CONCLUSIONS: We conclude that the combination therapy of diabetic rats with lower doses is still as efficient as higher doses; therefore, the way forward for reducing complications in long term consumption.

Cardiovascular diseases (CVDs) identified as a serious public health problem. Although there is a lot of evidence that inflammatory processes play a significant role in the progression of CVDs, however, the precise mechanism is not fully understood. Nevertheless, recent studies have focused on inflammation and its related agents. Nucleotide oligomerization domain-, leucine-rich repeat-, and pyrin domain-containing protein 3 (NLRP3) is a type of pattern recognition receptor (PRR) that can recognize pathogen-associated molecular patterns and trigger innate immune response. NLRP3 is a component of the NOD-like receptor (NLR) family and have a pivotal role in detecting damage to cardiovascular tissue. It is suggested that activation of NLRP3 inflammasome leads to initiating and propagating the inflammatory response in cardiomyopathy. So, late investigations have highlighted the NLRP3 inflammasome activation in various forms of cardiomyopathy. On the other side, it was shown that noncoding RNAs (ncRNAs), particularly, microRNAs, lncRNAs, and circRNAs possess a regulatory function in the immune system's inflammatory response, implicating their involvement in various inflammatory disorders. In addition, their role in different cardiomyopathies was indicated in recent studies. This review article provides a summary of recent advancements focusing on the function of the NLRP3 inflammasome in common CVDs, especially cardiomyopathy, while also discussing the therapeutic potential of inhibiting the NLRP3 inflammasome regulated by ncRNAs.

This study evaluates the synergistic effect of garlic and Citrullus colocynthis on diabetic reproductive damage by suppressing the AGEs/RAGE/Nox-4 signaling pathway. Thirty-five male Wistar rats were divided into five groups (n = 7/group): Control, Diabetic, Diabetic+G (Garlic, 1 mL/100 g b.w), Diabetic+C (C. colocynthis, 10 mg/kg b.w) and Diabetic+GC (Garlic, 1 mL/100 g b.w and C. colocynthis, 10 mg/kg b.w) groups. At the end of the experimental period (30 days), in diabetic rats, glucose increased, and body & testis weight, luteinizing hormone (LH) and testosterone levels, and sperm count decreased significantly and histopathological injuries were observed. In addition, they have increased testicular apoptosis and oxidative stress. Also, the mechanism based on advanced glycation end products (AGEs)/receptors for advanced glycation end products (RAGE)/NADPH oxidase-4 (Nox-4) was activated in diabetic rats. Separate consumption of garlic and C. colocynthis in Diabetic+G and Diabetic+C groups alleviated the negative adverse effect of diabetes to some extent, but when they were used in the combination form (Diabetic+GC) improvement was profound. Testis histopathology, increased body and testis weight, and enhanced capacity in protecting diabetic reproductive injury was seen. Decreases in testosterone and LH concentration and sperm count in diabetic rats were also reversed by combined administration of garlic and C. colocynthis. It regulated oxidative stress markers, meanwhile reducing caspase-3 immunoexpression. In addition, overexpression of RAGE, Nox-4 and nuclear transcription factor-κB (NF-κB) was inhibited by the combination of garlic and C. colocynthis. PRACTICAL APPLICATIONS: Diabetes mellitus is wide spread all around the world with variety of complications in body including reproductive system in which patients suffer from physical and psychological aspects. Despite many efforts in providing agents for controlling diabetes and its complications, economic conditions of some countries make it difficult for people to provide costly medicine and as a result, they have to bear the complications until they pass away. However, traditional medicine is still finding its way, especially in poor countries with emphasis on medicinal plants. There have been many studies on plants to alleviate diabetes or its side effects. But, using one plant for long term, may be not so effective. Here, we attempted to find whether two plants from two different species can show more efficacy than each one alone. We noticed garlic and Citrullus colocynthis despite having beneficial effects when used alone, they could show synergistic effects in combination.

Background: In Iranian traditional medicine, Rydingia persica (R.P) is commonly used to treat diabetes mellitus (DM). Objective: We assessed the protective effects of R.P against testis and epididymis oxidative stress and the hormonal changes induced by DM. Materials and Methods: Forty male Wistar rats (12 wk old) weighing 230-270 gr were divided into five groups (n = 8/each): 1. Control (C); 2. diabetic (D); 3. diabetic + R.P200 (D+R200); 4. diabetic + R.P400 (D+R400); and 5. diabetic + R.P600 (D+R600). Groups C and D received 2 ml of normal saline orally daily for two wk and groups D+R200, D+R400, and D+R600 received 200, 400, and 600 mg/kg body weight of R.P powder, respectively, orally daily for two wk. DM was induced by a single intraperitoneal injection of streptozotocin at 60 mg/kg body weight. We assessed malondialdehyde, glutathione peroxidase, glutathione reductase, superoxide dismutase, catalase, hydrogen peroxide, and glutathione in both the testis and epididymis and also the histological changes of the testis. Results: Diabetic rats showed a significantly increased and decreased level of oxidant and antioxidant factors, respectively, and a significantly lower level of serum testosterone and luteinizing hormone than the control group. In the histological study of the testis, deteriorations were observed. Treatment with R.P reversed these changes toward the state of the control group with the highest effectiveness shown by group D+R600. Conclusion: The data obtained suggest that R.P powder has antioxidant effects on testis and epididymis tissues in diabetic rats and that it improves histological testicular structure in diabetics. It can also correct testosterone and luteinizing hormone changes induced by DM.

BACKGROUND: In this trial, additive effects of acupuncture to selective serotonin reuptake inhibitors (SSRIs) treatment for reducing anxiety, were investigated. METHODS: 112 patients with anxiety disorder were randomly divided into three groups including SSRIs alone (drug group), SSRIs with sham acupuncture (control group) and SSRI with acupuncture (acupuncture group), and treated for 4 weeks. At the beginning of the study and on day 28, Spielberger State-Trait Anxiety Inventory (STAI) questionnaire was completed and serum levels of cortisol were measured. RESULTS: 105 patients completed the treatment period. STAI score showed significant differences among the three groups at the end of the study; importantly, changes in STAI score in the acupuncture group were significantly larger than the other groups. A decrease in cortisol levels was observed in all groups; though statistically non-significant, this decrease in the acupuncture group was larger. CONCLUSION: Acupuncture combined with SSRIs can significantly improve anxiety state compared to anti-anxiety therapy using SSRIs alone.

BACKGROUND: Background: Oxidative stress plays an important role in the development of diabetic complications. OBJECTIVE: This study evaluated the impact of pre- and post-treatment with Lactobacillus acidophilus ATCC 4356 on the oxidant and anti-oxidant factors of testis and epididymis in streptozotocin-induced diabetic rats. MATERIALS AND METHODS: Thirty male Wistar rats (10 wk old) weighing 220-230 g. were divided into five groups (n = 6/ each): 1- normal group, 2- normal lactobacillus group, 3- diabetic group, 4- diabetic + lactobacillus before (DLB) group, and 5- diabetic + lactobacillus after (DLA) group. The normal and diabetic groups received daily 1 mL normal saline for 6 wk. Normal lactobacillus group received daily L. acidophilus for 6 wk. Group DLB received daily L. acidophilus for 2 wk before diabetes and for 4 wk after diabetes. Group DLA received daily 1 mL normal saline for 2 wk before diabetes and L. acidophilus for 4 wk after diabetes. The dose of L. acidophilus was 1 × 10 9 CFU/mL. RESULTS: The administration of L. acidophilus worsened blood glucose level and reduced the levels of Malondialdehyde (p ≤ 0.0001) and Hydrogen peroxide (p ≤ 0.0001) and, Catalase and Glutathione peroxidase activity increased in the testis. In epididymis, Glutathione peroxidase and Catalase (p = 0.013) activity increased and Hydrogen peroxide concentration reduced, while Malondialdehyde concentration did not show any changes compared to the diabetic rats. Also, there was no significant difference between DLB and DLA groups, in these markers. CONCLUSION: Data obtained suggests that L. acidophilus has anti-oxidant effects on the testis and sometime in the epididymis in diabetic rats.

BACKGROUND: Diabetes mellitus is one of the most common metabolic diseases in humans, affecting 100 million people around the world. OBJECTIVE: Investigating the effects of Citrullus colocynthis pulp on oxidant and antioxidant factors of testes and epididymis in streptozotocin-induced diabetic male rats. MATERIALS AND METHODS: Thirty two male rats were divided into four groups (n=8) 1) N (normal) group, 2) N+C group, 3) D (diabetic) group and 4) D+C group. Groups N and D received normal saline 2 ml orally for two weeks and groups N+C and D+C received 10 mg/kg.bw Citrullus colocynthis pulp orally for two weeks. Diabetes was induced by single intraperitoneal injection of streptozotocin (STZ) at 65 mg/kg. RESULTS: D group had a significant increase in H2O2 (Hydrogen peroxide) and MDA (malondialdehyde) concentrations, and CAT (catalase) activity, and also a significant decrease in Peroxidase (POD) activity compared to N group. D+C group had a significant decrease in H2O2 and MDA concentrations and, CAT activity and significant increase in POD activity compared to D group. CONCLUSION: Citrullus colocynthis pulp in two weeks had beneficial effects on oxidants and antioxidants changes in reproductive system in streptozotocin-induced diabetic rats.

RESUMO

Four species of eriophyoid mites are recorded from Iran, of which three represent new records and the other is a new species. These species are: Brevulacus salicinus n. sp. (Diptilomiopidae), found on Salix sp. (Salicaceae); Aceria wallichianae Keifer, 1975 from Ulmus minor (Ulmaceae); Aceria granulata Carmona, 1972 from Verbascum spp. (Scrophulariaceae) and Tegnacus unicornutus Pye, 2012 from Carpinus betulus (Betulaceae). Each of these species are illustrated and provided with data regarding their distribution and host plants.

INTRODUCTION: Health literacy refers to the degree to which individuals have the capacity to obtain, process, and understand the basic health information needed to make appropriate health decisions. The aim of this study was to determine the health literacy level in regions on the outskirts of Mashhad, Iran. METHODS: This cross-sectional study was undertaken in December 2014 on residents in the outskirts of Mashhad. We used a multistage sampling method, and data were collected via a questionnaire, conducted by trained interviewers. The data were analyzed using the t-test, the Mann-Whitney test, and the chi-squared test. RESULTS: We had 502 participants in this study. The mean age was 35.0 ± 11.06 years. Sixty percent of participants (301) were female, and 86.3% (425) had high school diplomas or lower educational levels; the rest had higher educational levels. Television/radio and the Internet were the most and least frequently accessed and used media, respectively. There were significant correlations between the level of knowledge and response to questions in specific domains, such as weight, fasting blood sugar, and blood pressure (p < 0.001). The participants had no difficulty reading booklets (63%), physicians' prescriptions (59%), medical forms (56%), and guidance boards in hospitals (71%). People had no problems understanding most health/disease materials obtained via various routes (37%), but the health/disease materials published on the Internet/electronic resources (37%) and textbooks (64%) were difficult for them to understand. CONCLUSION: Health literacy is a complicated construct, and more comprehensive studies are needed to develop health-related information that can be understood by more of the general public.

Better postoperative management of patients who have undergone single ventricle (SV) Fontan procedure could potentially reduce long-term complications and improve the quality of life for patients. The present study determined the effect of tadalafil on myocardial and endothelial function and exercise performance after modified Fontan operation. Patients who had undergone SV modified Fontan operation were enrolled in this clinical trial. The demographic characteristics of the patients were recorded. Before administration of tadalafil and after the trial, ventricular function (MPI, EF, FS, E/A, VTI), exercise performance, and endothelial function were evaluated for sonographic and biochemical markers (FMD, IMT, ICAM, VCAM, NO) using echocardiography, exercise testing, vascular ultrasonography, and biochemical measurements, respectively. A single dose of tadalafil of 1 mg/kg was administered daily for 6 weeks, and the functional class of the patients before and after tadalafil was determined. A total of 15 patients completed this clinical trial. Tadalafil was shown to have a significant effect on myocardial function, exercise performance, and improvement in NYHA functional class (p < 0.05) of study population. It had no significant effect on the biochemical variables and endothelial function except for IMT (p > 0.05), which decreased significantly after tadalafil administration (p < 0.05). The findings indicate that tadalafil is a safe, well-tolerated agent for the use after modified Fontan operation to improve myocardial function and exercise performance and possibly reduce long-term morbidity and mortality of patients. More conclusive results could be obtained from further study with a larger sample size and long-term follow-up.

BACKGROUND: Tachycardia-induced cardiomyopathy (TIC) is a rare cause of dilated cardiomyopathy (DCMP). The diagnosis can be missed because tachycardia is a common symptom in DCMP. CASE REPORT: We reviewed a case 5-year-old with palpitation and dyspnea with symptoms and signs of heart failure that diagnosed as DCMP initially. Then, in the evaluation for cause of tachycardia, atrial tachycardia was detected. Hence, treatment with flecainide was started and after 3 months, left ventricular (LV) systolic function and symptoms of the patient was relieved. CONCLUSION: TIC should be suspected in all patients with unexplained LV dysfunctions in the setting of a persistent tachyarrhythmia.

Antidiabetic action of garlic is established in animal studies. Since all of the pervious studies have focused on the therapeutic role of garlic, this study investigated the preventive effect of garlic juice on biochemical factors and histological features in Streptozotocin (STZ)- induced diabetic rats. Forty male rats were divided into five groups (n = 8): 1-Normal group (N), 2-Normal+Garlic group (N+G) received garlic juice (1 mL/100g BW) for 6 weeks, 3-Diabetic group (D) was injected with STZ (60 mg/kg, IP), 4-Diabetic+Garlic-before group (D+Gb) received garlic juice for 3 weeks before STZ injection and continued for another 3 weeks, 5-Diabetic+Garlic-after group (D+Ga), three days after STZ injection, they received garlic juice for 3 weeks. Serum biochemical factors were measured by the enzymatic methods and H&E stained sections of pancreas and liver were prepared for light microscopy. In diabetic rats, elevated levels of glucose, cholesterol and triglycerides, the increment of the activities of ALT and AST, increased food and water consumption were observed. The abnormal increases were significantly (p < 0.05) decreased in D+Gb groups compared to D group. In D group, scattered degeneration of the hepatocytes with lymphocytic infiltration in the portal areas, decrease of pancreatic islets numbers and diameter, atrophy of pancreatic islets were observed. These abnormal histological signs were dramatically ameliorated in D+Gb group compared to D group. In D+Ga group compared to D+Gb group slighter effects of garlic juice on histopathological and biochemical changes were seen. These results indicate that garlic juice may help in the prevention of the complications of diabetes.

INTRODUCTION: Renal reperfusion injury is associated with increased mortality and morbidity due to acute kidney failure. Oxidative stress induced with renal reperfusion affects glomeruli and tubular epithelium through reactive oxygen species; therefore, the use of medicinal plants appears rational for improvement of reperfusion effects. The aim of present study was to examine the preventive effect of garlic juice (Allium sativum) on renal reperfusion injury in rats. MATERIALS AND METHODS: A total of 30 male Wistar rats were divided into 5 groups: control, garlic, sham (right nephrectomy), reperfusion, and reperfusion + garlic groups. After right nephrectomy, renal ischemia and reperfusion were induced. At the end of the experiment, all rats were killed and kidney function tests and histopathological examination were performed. Results. Reperfusion increased serum urea and fractional excretion of sodium levels, while it decreased urine potassium levels and creatinine clearance. However, garlic juice significantly decreased serum urea levels in the reperfusion + garlic group compared with the reperfusion group (P < .001). Preteatment with garlic juice also resulted in significant increase in urine potassium (P = .03) compared to reperfusion. Fractional excretion of sodium and creatinine clearance were also improved. On histological examination, rats pretreated with garlic juice had nearly normal morphology. CONCLUSIONS: The results of this study showed that garlic juice significantly prevented renal reperfusion-induced functional and histological injuries.
